Below is a listing of forms that you may need to complete to get started on your travel or per diem assignment:
I. License and Certifications:
- Valid and current Professional License for the state of your assignment (valid for at least the whole duration of your assignment)
- CPR/BLS/BCLS (Required for all healthcare professionals. Must be issued by American Heart Association. Must include the front and the back of the card. The card must be signed by you. Must be basic life support for healthcare providers or equivalent)
- ACLS (Required for Critical Care, L&D, ER and most Telemetry/Step Down units. Must be issued by American Heart Association. Must include the front and the back of the card. The card must be signed by you.)
- PALS (Required for Pediatrics, PICU, and ER units. Must be issued by American Heart Association. Must include the front and the back of the card. The card must be signed by you.)
- NRP/NALS (Required for L&D, Nursery, and NICU units. Must inclue the front and the back of the card. The card must be signed by you.)
- Fetal Monitoring Certificate (required for L&D)
- TNCC (May be required for ER units)
- Fire Card: Los Angeles County Assignments only (required within 30 days of starting assignment)
II. Health Documentation:
- Physical or Physician’s Statement (Must be performed within the last 12 months. Some facilities may required the physical be performed within 30 days of the start date. Physical must state that you can work without restrictions and are free of communicable diseases.)
- Medical Release form
- PPD or Negative Chest X-Ray ( Be ware of the fact that some facilities may require a 2-step PPD. Some facilities may require people with a history of a positive PPD to have a new chest x-ray within 30 days of the assignment.)
- TB Questionaire (only required if you have a positive PPD)
- Mumps Titer (some facilities may accept vaccination records in lieu of titer)
- Rubeola (Measles) Titer (some facilities may accept vaccination records in lieu of titer)
- Rubella Titer (some facilities may accept vaccination records in lieu of titer)
- Varicella Titer Lab Slips (some facilities may accept vaccination records in lieu of titer)
- Hep B Form (some facilities may require a titer)
- TB Mask Fit Test (if required by facility)
III. Other Important Documents:
- Drug Test (Required prior to the start of your assignment)
- Housing Documents (only if housing is provided by ATC Global Nurses)
- Benefits Documents (for those opting to get benefits)
- Employee Handbook Manual-Signed
- Job Description Acknowledgement
- Safety Manual and HIPPA Acknowledgement
- Annual Safety orientation manual acknowledgement
- Medication Test: Required of all LVNs and RNs (80% score is required)
- Competency exam (links to Nursetesting.com)-80% score required
- Release Authorization
- Any Other documents that may be required by the Healthcare Facility
